Note that submission of this information ONLY registers your interest in joining the League
In order to SECURE your membership, you MUST Print out, Sign, and remit the Media Waiver
along with your Dues (checks only please) to the Director of the League once membership is
opened for the season.

The date checks are received is the date upon which a position in the League will be permanently reserved for you. Only individuals who complete this online form AND remit the Waiver
ALONG WITH their payment will have a position reserved in the League for the 2008 season.
